Great tool! I used it for my appointment this morning. It took a couple of minutes to fill in the online form and less than 20 seconds to go through screening.


You can do your COVID-19 screening for your upcoming outpatient appointment online with the mobile screening tool.

The mobile screening tool, available in English and French, only takes a few minutes to complete and patients will receive an email with confirmation to bring with them, along with their appointment slip, to the hospital in printed form or on their mobile device.

Screening staff will validate the confirmation at entry and you will be able to go directly to your appointment.

This mobile screening must be completed a maximum of four hours before your appointment. If the screening confirmation expires, you will need to re-do the mobile screening or be screened in person (by a screener) when you arrive.

With the exception of caregivers for children, this mobile screening tool is for patients only and does not give family members or caregivers an option to pre-screen because we must continue to restrict family presence for outpatients at KHSC in order to maintain physical distancing.

We are proactively screening everyone who enters our Hotel Dieu Hospital and Kingston General Hospital sites. Screening helps us quickly identify patients and staff who have signs and symptoms or are at risk of having a COVID-19 infection. Please come a few minutes early for your scheduled procedure or appointment as it takes a few minutes to go through the screening process when you enter.

Registered family members must present photo identification that clearly shows face and complete first and last name.

For booked appointments, please have your appointment slip ready.

What will I be asked at screening?

You will be asked the following questions when you arrive at the Hotel Dieu Hospital or Kingston General Hospital site:

  • Do you have a fever (temperature of ≥ 37.8 C)?
  • Do you have any of the following symptoms?
    • Cough

    • Sore throat

    • Shortness of breath

    • Chills

    • Changes in your sense of taste/smell

    • Runny nose, sneezing, nasal congestion (not due to other underlying reasons like seasonal allergies or post-nasal drip)

    • Difficulty swallowing

    • New headache

    • Unexplained fatigue/malaise

    • Nausea, vomiting, diarrhea, abdominal pain

  • Have you been tested for COVID-19 due to symptoms and are awaiting results? (Asymptomatic/Surveillance testing does not apply and is not a reason for exclusion.)

  • Have you tested positive for COVID-19 in the last 20 days?

  • In the last 14 days, have you:

  • In the past 14 days, have you had close**, unprotected contact with any of the following:
    • ​​​a  sick traveler
    • a suspected*** or confirmed case of COVID-19?
  • Visitors ONLY: Have you read, and do you agree to abide by, the KHSC Guidelines for Families visiting Loved Ones during COVID-19?

*Visited - spent greater than 15 minutes indoors or within an enclosed space

**Close, unprotected contact is defined as having provided care, had similar close physical contact, or had lived with or otherwise had close, prolonged contact without appropriate PPE.

***Asymptomatic or Surveillance testing in one of your contacts does not apply.

****Red and Grey zone as per provincial framework.


What if I answer yes to any of these questions?

Help us keep our staff and other patients safe by answering honestly and keeping in place any mask you are given by our screening staff until you are told to remove it by your nurse or doctor.

If you are a patient and you answer yes to any of these questions, you will be provided with a mask and you can proceed to your appointment where we will discuss your care options with you.

Visitors and support persons who answer yes to any of the questions are not permitted to enter the building.
