Stroke Prevention Clinic

The Stroke Prevention Clinic provides specialty prevention services for people who are at risk of having a stroke and those who have experienced a transient ischemic attack or a stroke. People who are referred have their conditions assessed for level of urgency and are supported to reduce their risks accordingly. The clinic takes an interprofessional team approach to care. Services are provided by experienced stroke neurologists, a clinical nurse specialist, and a registered dietitian, with support from medical secretaries.

Monday to Friday 8 a.m. - 4 p.m.

Referrals

Patients require a referral from their family doctor or primary-care provider, an emergency department physician, or another specialist. Patients who have been admitted to the hospital with a stroke are also referred for secondary prevention follow-up in this clinic. Health-care professionals can access our referral form here.

Services

  • Interprofessional team care
  • Timely assessment of conditions for level of urgency and for stroke risk
  • Review of diagnostic tests
  • Medical management to reduce stroke risk
  • Referral for surgical management if needed
  • Patient and family education
  • Counselling in clinical nutrition and healthy eating
  • Referrals to other specialists as warranted (e.g., diabetes, cardiac specialists, etc.)
  • Referrals to other allied health services (e.g., home care or rehabilitation)
  • Navigation to community supports including driving assessment
  • Follow-up services from inpatient stroke care
  • Communication with the primary-care provider

Preparation

There is no specific preparation needed but please remember to bring:

  • your health (OHIP) card
  • a complete, current list of your medications provided to you by your pharmacist OR all current medications in their original drugstore container (includes insulin*, inhalers, prescribed creams or eye drops, non-prescription medications or supplements such as herbals and vitamins)
    *You do not need to bring insulin to the clinic but please provide the following information: name of insulin, dose and frequency

What you can expect

  • You may be seen in person or through a phone visit/e-visit for follow-up visits
  • Your appointment can last up to 30 minutes
  • You may be asked to do bloodwork or other tests
  • Please be sure you have safe transportation to and from the clinic
  • Kingston Health Sciences Centre is a teaching hospital and as such there may be student physicians who are involved in your care.
