Eating Disorders Programs

Referrals for all eating disorders programs, including the Eating Disorders Day Treatment Program and Child and Youth Eating Disorders Program.

Referrals are accepted from physicians and nurse practitioners. Please include a completed referral form, recent blood work and an ECG.

Adult Outpatient Mental Health Program (not including the urgent mental health stream)

A physician referral is required for this program. Referrals for this program can be submitted through AccessMHA.

Direct referral forms

A patients or primary care providers can submit a direct referral the Adult Outpatient Mental Health Program, instead of using AccessMHA.  Please note that, effective April 1, 2025, we will no longer accept direct referrals and all referrals must be made through AccessMHA.
