Effective immediately masking is required for everyone when present on all inpatient units, in the Emergency Department (ED), the Urgent Care Centre (UCC), and the Children’s Outpatient Centre (COPC).
Integrated care pathways for chronic disease
For people with chronic disease, integrated care pathways are a foundation for advancing population health. They are integral to enhancing population health by preventing and managing chronic diseases. KHSC will work with partners from the FLA OHT, creating a common pathway to manage two prevalent chronic diseases – chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF). These pathways will start before people enter a hospital and provide evidence-based, best-practice integrated care guidelines for each step of their journey. This approach is wrapped around care for the people we serve while ensuring individuals access to care before they become severely ill.
