The Frontenac, Lennox & Addington Ontario Health Team (FLA OHT) aims to connect providers in primary care, hospitals, specialty clinics, home care, community paramedics, pharmacists and others in the FLA OHT, to create a common pathway to manage prevalent chronic diseases. These pathways will provide evidence-based, best-practice integrated care guidelines for each step of their journey. This approach emphasizes preventative care and early chronic disease management.
