Care management pathways are integral to enhancing population health by preventing chronic disease and managing specific conditions in the community. The pathway documents below have been developed in partnership with primary care, through the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) and help ensure people have access to care before they become severely ill and enter a hospital or need to see a specialist. Each document focuses on diagnosis, management and patient referrals within the health system.
